Collect answers with a form

Needs Starter (3 forms) or Pro and Max (no cap). Quizzes and applications count toward the 3 too.

Start from a template

Click New → Start from a template:

• Client onboarding form: what you need from a new client before you start.
• Feedback survey: what buyers liked, and your next testimonial.

Or click New → Build with AI, pick Form, and choose Get feedback. Or click Something else and type your own goal.

Add questions

Click Add question and pick a type:

Type	What people do
Single choice, Multiple choice	Tap one answer, or a few
Picture choice	Tap a picture
Yes or no	Tap one of two big buttons
Scale	Pick a number, like 1 to 10
Short text, Long text	Type a line or a few
Email, Name	Type their email or name
Statement	Read a line and click Continue

Turn Required off to let people skip a question. Write {name} in a question to use the name they gave.

Know who answered

Forms don't ask for an email by default. Click Email ask and set Ask for an email to Required or Optional.

To pick which lists those emails join, use Send signups to under Email ask. Out of the box it follows your store default. See Connect your email list.

The thank-you screen

Click Thank-you screen to change what people see at the end. Add a Next step (optional) button, like a link to book a call or your store.

Tag everyone who gets this tags everyone who finishes the form and gives an email, so you can reach them later.

Share it

Click Publish. Your link looks like yourname.eden.so/form/client-onboarding. Add it to your page with Add block → Form, or send it to clients by email.

See the answers

Open the form and click Responses. Click a row to read every answer. Click Export CSV to download them all.
